Our Services for Adults with Developmental Disabilities

Residential Support services to enhance your quality of life based on your individuality and choice. We serve individuals with intellectual and developmental disabilities to include: Autism Spectrum Disorder, Cerebral Palsy, Fetal Alcohol Spectrum Disorder, Down Syndrome, and Attention Deficit Dyperactivity Disorder (ADHD). Cozy Horizons Supported Living is a Virginia Medicaid Waiver provider. Waivers for Family and Individual Supports (FIS), Community Living (CL) and Building Independence (BI), cover Supported Living, In-Home Supports and Independent Living.

Supported Living Residential

Our Adult Supported Living Program provides residential support for adults with intellectual and developmental disabilities. Also referred to as Supervised Living, this is a great solution for adults seeking more independent residency outside of their parents home or a group home. Cozy Horizons will provide the supports that enable the person supported to acquire, retain, or improve skills necessary to reside in their home. This could be an individual’s sole residence or a residence shared with roommates of like interest. Supported Living service is covered in the waivers for Family and Individual Support (FIS) and Community Living (CL).

In-Home Support Service

Our In-home support provides residential support for adults with intellectual and developmental disabilities. This service is a residential service that takes place in the individual's home or family home. This service supplements the primary care provided by the individual, family, or other unpaid caregiver. It is designed to ensure the health, safety, and welfare of the individual. In-Home Support Service is covered in the waivers for Family and Individual Support (FIS) and Community Living (CL).

Independent Living

Independent living support service means a service provided to adults 18 years of age and older that offers targeted skill building and supports necessary for individuals to secure and maintain their own home in the community. An individual receiving this service typically lives alone or with roommates in the individual's own home or apartment. The supports may be provided in the individual's residence or in other community settings. Independent living support service shall be covered in the Building Independence (BI) waiver.
