Independent Living Residential Service (Northern Virgina)
Our Independent Living service for adults in Northern Virginia provides residential support for adults with intellectual and developmental disabilities. Many individuals with developmental disabilities have chosen to live in regular homes of their own, with less restrictive settings. They may need ongoing supports and services from others - which Cozy Horizons is here to provide!
This is not a Group Home Service.
Independent living support service is a service provided to adults 18 years of age and older that offers targeted skill building and supports necessary for individuals to secure and maintain their own home in the community. An individual receiving this service typically lives alone or with roommates in the individual's own home or apartment. The supports may be provided in the individual's residence or in other community settings. Independent living support service shall be covered in the Building Independence (BI) waiver.
We create and maintain a network of trusted support with seasoned professionals. The level and extent of Independent Living Services required for each individual varies greatly, and are tailored for each in their individual plan.
This service takes place in a residential setting operated Cozy Horizons:
Individuals choose an apartment or home that is subleased from Cozy Horizons, using a rent subsidy.
Individuals have the choice to live alone or with roommates of similar interests.
The Supported Home Living services we offer are extremely flexible.
Services and support are available 24/7.
Also referred to as Supervised Living, this is a great solution for adults seeking more independent residency outside of their parents home or a group home. Cozy Horizons will provide the supports that enable the person supported to acquire, retain, or improve skills necessary to reside in their home.
All individual goals and objectives, along with needed supports will be established through the person-centered planning process and will include opportunities to seek employment and work in competitive integrated settings, engage in community life, and control personal resources, as applicable based on the needs and preferences of the individual.
Allowable activities include the following:
Skill building and supports necessary to promote the individual's stability in the individual's own home and community in the absence of a primary caregiver living in the residence;
Skill-building and supports to promote the individual's community participation and inclusion in meaningful activities;
Skill-building and supports to increase the individual's socialization skills and maintain relationships;
Skill-building and supports to improve and maintain the individual's health, safety, and fitness, as necessary;
Skill-building and supports to promote the individual's decision-making and self-determination;
Skill-building and supports to improve and maintain, as needed, the individual's skills with ADLs and IADLs;
Routine supports with transportation to and from community locations and resources; and
General supports, as needed.
Cozy Horizons supported living is licensed through the Virginia Department of Behavioral Health and Development Services (DBHDS). Serving adults 18 years and older.
We accept Private Pay individuals as well as individuals covered under the Virginia State Developmental Disabilities (DD) waivers:
